General Summary of Position
The Population Health Equity Coordinator will play a vital role in implementing strategies to advance health equity goals and implementation and coordination of population health programs within MedStar Family Choice. This position involves collaboration with internal teams, healthcare providers, and community partners to create an environment that prioritizes equity and inclusivity in all aspects of healthcare and ensures the effective delivery of population health services and improvement of health outcomes for our member population.


Primary Duties and Responsibilities

  • Assist in planning, implementation, and coordination of population health programs and interventions.
  • Coordinate the implementation of health equity programs and initiatives within the health plan, ensuring alignment with the organizational objectives.
  • Collaborate with cross-functional teams to integrate health equity principles into the health plan's service delivery model.
  • Collect, organize, and analyze population health data to identify trends, disparities, and opportunities for intervention.
  • Collect and analyze data related to health disparities and social determinants of health.
  • Utilize data findings to inform the development of targeted interventions and initiatives to reduce disparities.
  • Contribute to the development of reports and presentations based on data findings.
  • Establish and maintain relationships with community organizations, advocacy groups, and underrepresented populations to better understand their healthcare needs.
  • Collaborate on community engagement initiatives to promote health education and awareness.
  • Work closely with healthcare providers to ensure culturally competent and equitable care delivery and seamless integration of population health initiatives into clinical care.
  • Advocate and support initiatives that enhance provider cultural competence and inclusivity.
  • Provide administrative support for population health and health equity initiatives, including scheduling meetings, maintaining documentation, and tracking metrics.
  • Collaborate with relevant departments to streamline administrative processes.
  • Participate in quality improvement initiatives related to population health outcomes.
  • Contribute to the development and monitoring of key performance indicators (KPIs).
  • Develop and implement training programs for internal staff to enhance cultural competence and inclusivity.
  • Work with Human Resources to integrate diversity and inclusion principles into organizational policies and practices.


  • Minimum Qualifications
    Education

    • Bachelor's degree Public Health, Health Policy, or a related field required and
    • Master's degree Public Health, Health Policy, or a related field preferred

    Experience

    • 3-4 years health equity, community health or a related field required and
    • HEDIS experience, understanding of health disparities, cultural competence, and social determinants of health preferred

    Knowledge, Skills, and Abilities

    • Excellent verbal/written communication and organizational skills.
    • Basic computer skills.
    • Capable of working independently as well as part of a team.
